Gabapentin's reputation is built almost entirely on comparison. What The Research Found
Researchers used the TriNetX database to track more than 52,000 adults with chronic low back pain over 10 years, comparing patients prescribed gabapentin against a closely matched group who weren't, matched on age, health conditions, and other pain medications, specifically to isolate gabapentin's association from the general risks that come with chronic pain itself. Across the full sample, gabapentin use was linked to a 29% higher risk of dementia and an 85% higher risk of mild cognitive impairment (MCI).
The sharper finding was in adults under 65. In that group, dementia risk was 110% higher and MCI risk 150% higher, with the steepest rise concentrated in patients aged 35 to 49, a population not typically flagged for either condition. The study also found a dose-response pattern: patients with 12 or more gabapentin prescriptions had a 40% higher dementia risk and 65% higher MCI risk than those with only 3 to 11. This is an observational study, and it did not control for treatment duration or the actual dose taken, only prescription count, so it establishes a strong association, not proof of cause.
What This Means
How Gabapentin Quiets The Nervous System
Gabapentin works by binding to calcium channels involved in neurotransmitter release, which dampens excitatory activity in the nervous system. That's precisely why it helps with nerve pain, overactive pain signaling gets turned down. It's a targeted, well-understood mechanism, and it's the reason the drug works as intended.
Why The Same Mechanism May Cut Both Ways
The circuits responsible for pain signaling aren't fully separate from the circuits responsible for memory and attention, both depend on excitatory neurotransmission to function. Researchers suspect that dampening this activity for pain relief, sustained over years, may also interfere with the same signaling that memory and attention rely on. It's the same lever, pulled in one direction for one purpose, potentially producing an unintended effect elsewhere.
What The Dose-Response Pattern Adds
A single elevated risk number can be explained away by confounding factors. A graded pattern is harder to dismiss: more prescriptions consistently mapped onto more risk. That kind of dose-response relationship is one of the classic signals researchers look for when trying to distinguish a real biological effect from statistical noise.
Why Younger Adults Showed The Sharpest Rise
This is the most open question the study raises. Adults under 65 aren't typically at meaningful risk for dementia or MCI, which is exactly why a 110% increase stands out, there's less of a competing baseline risk to dilute the signal. It may reflect longer cumulative exposure ahead of them, differences in why younger patients are prescribed the drug, or a vulnerability the current data simply can't yet explain.
Practical Implications
None of this suggests gabapentin should be stopped abruptly or feared outright, for many people, it remains a reasonable tool for chronic pain. What it does suggest is that "safer than opioids" is a comparative claim, not a guarantee of being risk-free, and long-term prescriptions deserve the same periodic reassessment as any medication taken for years without a clear endpoint. That's a conversation for a doctor's office, not a reason for self-directed changes.
The Bottom Line
The real finding here isn't that gabapentin causes dementia, the evidence isn't there yet to say that. It's that a drug widely treated as the safe option has earned the same long-term scrutiny as the one it was meant to replace.
